CONSENT STATEMENT FOR THE PROCESSING OF PERSONAL DATA OF PATIENTS AND PATIENT'S RELATIVES

Dr. SİNAN AKYÜREK (data controller) may need to collect and store your personal information, sensitive personal data, and health data to provide you with the services we offer, within the limits required for the service to be provided.

Health data, which we must record to provide you with healthcare services, is considered special category personal data by law. In this context, according to Article 6, Paragraph 2 of the Personal Data Protection Law No. 6698, "the processing of special category personal data without the explicit consent of the data subject is prohibited." Therefore, personal health data can only be recorded with your explicit written consent, except in special circumstances defined by the law. Thus, it is mandatory to obtain your consent.

Accordingly, for the purpose of providing the services mentioned above, the personal and special category personal data specified in detail below will be recorded in accordance with the Personal Data Protection Law No. 6698 and other applicable laws if you give your explicit consent.

• Data that clearly belong to a specific or identifiable natural person, processed either partially or completely automatically or non-automatically as part of a data recording system, and contain information about a person’s identity, such as name, surname, national ID number, nationality, mother’s name, father’s name, place of birth, date of birth, gender, and documents like driving license, ID card, passport, tax number, social security number, signature information, etc.
  
  
• Data that clearly belong to a specific or identifiable natural person, processed either partially or completely automatically or non-automatically as part of a data recording system, and include information like phone number, address, email address, fax number, IP address, etc.
  
  
• Data that clearly belong to a specific or identifiable natural person, processed either partially or completely automatically or non-automatically as part of a data recording system, and include personal data related to records and documents taken upon entry to or during stay in a physical space, such as camera recordings, fingerprint records, and security point records.
  
  
• Data that clearly belong to a specific or identifiable natural person, including copies of personal documents that contain personal data, excluding those recorded under the scope of physical security information (photos and camera recordings).

• Data that clearly belong to a specific or identifiable natural person, processed either partially or completely automatically or non-automatically as part of a data recording system, including the data specified in Article 6 of the Personal Data Protection Law (e.g., health data, including blood type, biometric data, etc.).

Regarding the personal data recorded by us, in accordance with the European Council Convention No. 108, Article 8 of the European Convention on Human Rights, Article 20 of the Constitution, and the Personal Data Protection Law No. 6698:

a) You have the right to learn whether your personal data is being processed,

b) If your personal data has been processed, you can request information about it,

c) You can learn the purpose of processing your personal data and whether it is being used in accordance with that purpose,

ç) You can inquire whether your personal data has been transferred to a third party or institution, either domestically or abroad,

d) If your personal data has been processed inaccurately or incompletely, you have the right to request its correction,

e) You have the right to request the deletion or destruction of your personal data in accordance with the conditions specified in Article 7 of the law,

f) You can request that any corrections or deletions made under (d) and (e) be communicated to third parties to whom the personal data has been transferred,

g) You have the right to object to the conclusion of an unfavorable result about you through exclusively automated processing of your personal data,

ğ) If your personal data has been processed unlawfully and causes harm, you have the right to request compensation for the damage.

Method of Application to the Data Controller Dr. SİNAN AKYÜREK for Patient/Relative/Consultant Rights

If you, as a Patient/Relative/Consultant, have a request regarding your rights under Article 11 of the Personal Data Protection Law No. 6698, you can fill out the Personal Data Protection Application Form, which you can obtain from the Consultation, following the specified procedures, or apply with your application that meets the minimum conditions specified by the Regulation on the Procedures and Principles for Applications to Data Controllers, either by sending it to our email address drsinanakyurek@gmail.com with a mobile or electronic signature or by submitting it personally with a wet signature to “Harbiye Mah. Teşvikiye Cad. No:19/13 ŞİŞLİ/ISTANBUL” or via notary. We will process your application as "Dr. SİNAN AKYÜREK" and provide the result within the shortest time, but no later than thirty days, free of charge. However, if the process requires additional costs, the fee determined by the Personal Data Protection Board will be charged by "Dr. SİNAN AKYÜREK".

As a Patient/Relative/Consultant, I have read and understood the Enlightenment Text regarding the processing of my personal data by the data controller “Dr. SİNAN AKYÜREK”. In this context;

In accordance with the “Personal Data Protection Law No. 6698” and the “Regulation on the Processing and Protection of Personal Health Data,” I agree, declare, and undertake that my personal data, sensitive personal data, health data, including those that identify or can be used to identify me, and any data related to photos or videos, may be processed, stored, and shared by the data controller Dr. SİNAN AKYÜREK as per the scope defined in the Personal Data Protection Law No. 6698 and the Regulation on the Processing and Protection of Personal Health Data.
